Ways to Refer a Patient
- Call Consultation Center at 919-908-0000, or
- Complete and PRINT the Online Referral Form below; and fax to 919-596-6147 WITH REFERRAL NOTES.
- Fill out our Incoming Referral Form and fax to 919-596-6147.
- For providers referring male patients for a semen analysis only, or for female patients requiring an HSG only; please fill out our Incoming Referral Form and fax to 919-596-6147.
